The
New Covenant Board of Directors has always
kept a paper experience roster and used it
to review the experience of our members and
to determine those persons who have met the
criteria to be considered as Lay Directors.
We are establishing a new computerized date
base to make this information easier to
track. As we started to fill out the new
data base we realized some of our
information is out of date. We are asking
all members to fill out new forms so we can
be sure our data base is as current as
possible. Please fill out
the form as completely as possible.
This information will be used by future Lay
Directors when they are filling their
teams. All
information will be entered into the
Community Database. If you have a
change of address or other information that
needs to be updated it can be sent using
this form. This information is
considered confidential and will be used
only for the purpose stated.
